It relates to having actual sexual act in a form of sexual intercourse (penetration).
You lose your virginity once you have sexual intercourse.
What if I use a tampon?
It is very unlikely to break hymen (vaginal entry membrane) by using tampon.
In any case inserting it does not count as sexual intercourse (penetration) and you are still definitely a virgin.
Is it possible to tell if you are a virgin?
In both cases of boys and girls the answer is no. Virginity test is just to scare you.

Does it hurt to lose virginity?
Again, this is very individual. In some instances it does, in some it creates no problem.
It depends what your hymen is like. Usually it hurts at the beginning of penetration and after a little while pleasure takes over pain. Pain also depends on vaginal tightness.
Bleeding, caused by broken hymen, may or may not follow.
In more extreme cases it is not possible to penetrate a woman for many months. This may require a medical exam and possibly assistance. Fortunately it is very rare and it should not cause any worry as it can be corrected with a very simple procedure.
Women can also gradually stretch, if this is the case, using fingers or some suitable objects, till penetration is possible. It is a good idea to see your doctor. There is nothing wrong about it, no need to be shy.
Does it hurt if I’m uncircumcised when my foreskin retracts?
Boys are worried their penis would hurt especially when the skin retracts.
